Privacy statement
Here you can read which personal data Mondzorg Koningstraat processes, why this is necessary and what rights you have.
1. What data do we process?
We may process name, address, date of birth, contact details, insurance and payment details, appointment details and correspondence. When you become a patient, we also process data necessary for dental care, including health data, X-rays, treatment data, medication use and relevant medical history.
We process the data that you enter yourself via the website. Only enter information that is necessary for your question or registration.
2. Objectives and principles
We process data to answer questions, assess a registration and intake, schedule appointments, provide safe dental care, maintain a medical file, submit claims and comply with legal obligations.
Depending on the processing, the bases are: preparation or execution of the treatment agreement, a legal obligation, legitimate interest or consent. We process health data for the provision and management of healthcare and, before the establishment of a treatment relationship via a web form, on the basis of your explicit consent.
3. Is provision mandatory?
You choose whether you use a form. We cannot process your question or registration without the mandatory contact details. We only request medical information when this is necessary for safe and appropriate care.
4. Who do we share data with?
We only share data when necessary and permitted, for example with healthcare providers involved in the treatment, your health insurer, a factoring or billing party, dental laboratory, IT and hosting suppliers, or government agencies when required by law.
Medical data is not shared without a valid basis. We request permission for transfer to another healthcare provider when this is legally necessary. We make appropriate privacy and security agreements with processors.
5. Retention periods
In principle, a medical file is kept for at least twenty years from the last change, and longer if good care or the law so requires. We retain administrative and fiscal data for as long as the applicable legal retention obligation applies.
We do not store data from contact questions and unaccepted registrations for longer than necessary for handling, accountability and possible follow-up. We review this data periodically and delete or anonymize it when the purpose has expired.
6. Security
We take appropriate technical and organizational measures against loss, unauthorized access and unlawful processing. Access to medical data is limited to persons who need it for their work and who are bound by confidentiality.
7. Your Privacy Rights
You can request access, correction, restriction, transfer or deletion of personal data. The WGBO and legal exceptions also apply to medical files. You can always withdraw consent; this does not make previous processing unlawful and may mean that we cannot process a request further.

8. Complaint about privacy
If you have a privacy question, please contact us first. You also have the right to file a complaint with the Dutch Data Protection Authority via autoriteitpersoonsgegevens.nl.
9. Website and External Services
The website uses necessary technical resources and external suppliers for, among other things, hosting, fonts and icons. We do not use personal data from the forms for advertising without a separate valid basis. If analytical or marketing cookies are added later, we will request permission in advance where required by law.
10. Changes
We may adjust this statement if our working methods or the law changes. The current version and date are always on this page.
